Care Assistant - ECM Contract* ID - 15519

About the position

San Bernardino County is seeking a contracted Care Assistant to join the team at Arrowhead Regional Medical Center (ARMC). This role is integral to the Enhanced Care Management (ECM) program, which aims to provide comprehensive support to patients. The Care Assistant will be responsible for a variety of tasks that promote patient care and ensure effective communication within the healthcare team. The position requires a proactive approach to patient management, including assisting in rooming patients and recording vital signs during clinic visits.

 

If any vital signs are outside the normal range, the Care Assistant must escalate the situation to the ECM team, Registered Nurse Care Management (RNCM), or the primary care practitioner (PCP). In addition to direct patient care, the Care Assistant will conduct daily reviews of the inpatient census list and patient alerts in Care Director to identify patients who have had recent hospitalizations or emergency department visits. This will involve initiating the transition of care (TOC) workflow to ensure that patients receive the necessary follow-up care.

 

The role also includes maintaining effective communication with the IEHP Utilization Manager (UM) to relay information about newly identified patient needs, ensuring a closed-loop communication process. The Care Assistant will participate in Summary Case Record (SCR) meetings, serving as a scribe to document meeting notes and ensure that the meetings are productive and timely.

 

Building supportive relationships with patients is crucial, as it allows the Care Assistant to better understand their needs and challenges from the patient's perspective. This position is full-time, requiring approximately 40 hours of work per week, and involves travel throughout the County as needed. Candidates must be prepared to meet specific conditions of employment, including passing a background check and obtaining a valid Healthcare Provider level Basic Life Support (BLS) certification before starting the role.

Responsibilities

  • Assist in rooming and recording vitals of ECM patients during clinic visits.
  • Escalate care to ECM, RNCM, or PCP if vitals are outside of normal range.
  • Conduct daily scrub of inpatient census list and patient alerts in Care Director.
  • Initiate transition of care (TOC) workflow for patients with new hospitalizations or ED visits.
  • Ensure closed loop communication with IEHP Utilization Manager regarding patient needs.
  • Participate in SCR meetings, serving as scribe and documenting notes in the database.
  • Build supportive working relationships with patients to understand their needs and deficits.

Requirements

  • Successful completion of a Medical Assistant program, with proof required.
  • Six months of full-time work experience within the past two years assisting medical professionals in a medical environment.

Nice-to-haves

  • Two or more years of front and back office work experience in an acute care facility.
  • Bilingual candidates are encouraged to apply.
